My story

As a community-based geriatric social worker for the past 18 plus years, I’ve had the privilege of working with older adults and their families, providing care management services to ensure they can age in place comfortably and safely. My goal has always been to empower older adults to maintain their autonomy while connecting them with the resources and support they need to thrive at home.

This work became even more personal when I cared for my own aging parents, both of whom had dementia. Helping them age in place was a deeply meaningful experience, but it also underscored the complexities and emotional toll of caregiving. Balancing the needs of my own children, managing a full-time job, and providing care for my parents was overwhelming at times, yet it gave me firsthand insight into the unique challenges families face when navigating the aging process.

It was through these experiences, both personal and professional, that I realized the need for a more personalized, compassionate approach to care management. This is why I decided to start my own Geriatric Care Management practice. I wanted to provide families with the same guidance, support, and expertise that I had found so crucial for my own family. My practice is built on the belief that every older adult deserves to age with dignity, and that families should never have to face these challenges alone. Through my work, I strive to offer practical solutions, emotional support, and a steady hand during what can often be an overwhelming time.
